Parents,

We need updated physicals added to DragonFly for any player who will be returning to practice June 8th.

If your doctor’s office is not accepting appointments for physicals (as is the case for anyone with Kaiser Permanente insurance), Dr. Byars our team doctor will do the physical for you. His contact info and the various locations are below. Please ensure you let them know you are from Shiloh and your physical will be $50.

Attached also is a letter from Dr. Byars about the services he provides to Shiloh. The physical form can be found under the documents section of this website. If you scroll all the way to the bottom……the physical form can be found there under Documents.

The document Dr._Byars_letter.pdf was attached to this post.
